Anyone use a daily job log?

I'm curious if any of you use some sort of "Daily Job Log". Maybe a simple sheet that would include information like mileage, fuel and expenses, basic duties performed at specific jobs and the time it took, conversations with the customers, etc. I was thinking of integrating it in with the time sheet.

I have a notebook I use personally as a daily log and it has been very helpful to me to look back chronologically for information or notes regarding specific job info or conversations.

I was thinking about a foreman filling it out each day and we could keep it in a binder or something.

Anyway, just curious if any of you use something along these lines.

Thanks,
Bill

We use a log sheet for all of our crews. The form ask mileage, materials used for the day, description of task, and hours. It also has boxes for comments for the office and a box noting if the job was completed or not. We use this form for there time sheet as well, and have spots on the form that all foreman and crew members sign acknowledging the hours worked for payroll for that day. It has worked really well thus far.

We have a form that is filled out by the supervisor for each job each day with times, equipment, materials, subs and services, work description, notes and details of pesticide apps.
