change of order form

Could somewhat show me what a legal 'Change of Order' form looks like?

You can likely buy a whole packet of relevant forms from your local Office Depot or similar for $10-20.

What I have been doing is using the same old contracts/proposals.

Write in on top "Change Order"

As long as I have customer info and the contracts are dated and signed....I haven't had any problems.
