Lien Waiver form

Anyone have a short and sweet one you wouldn't mind me using?

I have a client that needs one (he got burned by a lndscaper a few years ago, so he's going into things more carefully this time). I can have him find one, but since I know we'll have to have one either way, I'd just as soon it be something favorable to me, rather than him.
