To the liability question - we don't ask for signatures on any waivers, but would be interested to hear other experienced voices on this topic.
We do build them according to the codes for that municipality, which I would expect affords some sort of liability protection.
That being said, I don't recall ever signing waivers for the fireplaces in the homes I've lived in. Or the stoves I've owned.
I've always wanted to, and maybe I'll have to include the following clause in our contracts:
